Please prepare the following documents and information for your health assessment registration:
- Request to undergo medical examinations with HAP ID.
- Original passport (currently valid or expiring within 2 years).
- Cost covers the initial health assessment process only. It does not include additional tests, specialist consultations and treatment, if required.
- Guardian’s ID card if the client is under 18 years old. Any client < 18 years old must be accompanied by a parent/ legal guardian.
- If you have a previous or existing medical condition, bring any medical reports, test results or prescriptions that you may have with you to your appointment.
Health assessment fees (effective from 01/10/2024):
Your health assessment fees will vary based on the health examination list associated with your HAP ID. The fees for each form are as follow:
- Physical Examination (form 501): USD 37
- Chest X Ray (form 502): USD 50
- Serum Creatinine and eGFR: USD 8
- IGRA test (form 719): USD 74
- HIV test (form 707): USD 9
- Hepatitis B test (form 708): USD 6
- Hepatitis C test (form 716): USD 6
