Please prepare the following documents and information for your health assessment registration:
U.S. Humanitarian Resettlement Program
- Form OF194 (Blue Sheet) issued by US Consulate or xMIF list from RSC BKK
- Original passport or ID if passport is not available
U.S. Immigrants Medical Assessments
- Interview Appointment letter (copy version) or PKT3-K (for K-visa)
- Original passport
- Resident address information in USA
- Client needs to visit IOM for health assessment before vaccination.
- Children <18 years old must be accompanied by parents or legal guardian.
- If you have a previous or existing medical condition, bring any medical reports, test results or prescriptions that you may have with you to your appointment.
Health assessment fees (effective from 01/10/2024):
- Under 2 years old: USD 210
- From 2 – under 15 years old: USD 284
- From 15 – under 18 years old: USD 334
- From 18 – under 25 years old: USD 389
- From 25 – under 45 years old: USD 339
- From 45 years old and above: USD 334
